LINK/HOME-BASED CRISIS INTERVENTION

The LINK Program provides intensive, short-term crisis intervention and stabilization, family education and service linkages to prevent psychiatric hospitalization. This program is designed for children ages 5-18 who live in Suffolk County. Our case managers work with the family to assess needs, facilitate referrals and linkages to support and treatment and are available 24/7.
